The team must confirm the appointment and what any individual assessment or proposed services involve. Ask about scope, fees and any record arrangements, including whether a workup, scan, simulation or other service is proposed and what it includes or costs.

## Bring the wording you want to understand

You can explain which name or document prompted your question and what matters to you in the decision. If you already have documents, ask the clinic which records are relevant and how they should be shared. This guide has no upload or record\-intake facility.
